Illness & Misadventure (Years 9-12)

Illness and misadventure provisions are available to support students who are unwell or have a valid reason that might prevent them from completing an assessment task. In any case of illness or misadventure relating to an assessment task, the affected student will need to make a formal application via the online form, for approval to do a postponed or substitute task.

Illness and Misadventure Applications must be submitted within 24 hours of the in-class assessment or exam.

Step 1

Years 11-12:

Complete and save at least one of the following supporting evidence forms (to be uploaded in Step 2):

Years 9-10:

If you are unwell, you do not need to attend a medical facility to get a Medical Certificate - you just need to complete and save the Statutory Declaration Form (to be uploaded in Step 2).

Step 2

Open the Covenant Illness and Misadventure Form and upload the relevant documents from Step 1 above by choosing 'Select Files' and adding your saved supporting documentation.

Step 3

Return to the Covenant Illness and Misadventure Form, complete and submit.
